Roof Inspection Contract

Midwest Construction agrees to provide the following services:

Schedule and provide an annual inspection with current condition report, recommendations and corresponding photos.

These services will be performed in accordance with the following fee schedule. 

Annual inspection including written report and digital photos: S695.00
The fee is restricted to the building or campus located at the defined address.
Any building or campus with a separate address will require a separate contract and fee.

Midwest Construction retains the right to cancel this contract at any time if terms and conditions cannot be or are not complied with.

Midwest Construction is an lnternational Code Council tested General Contractor. General liability insurance documentation wiil be provided on request.
All reports and recommendations are based on lCC standards. All inspections will be scheduled through listed contact person. Reports will be forwarded in written and digital and (flash drive) formats.

All on line orders will be printed in PDF format, signed and mailed to contact person.

All Covid restrictions and regulations wiil be strictry adhered to.
